Introduction : Apparait chez les sujets actifs (6-77/100.000/anne) en post-traumatique mal traitées, sur des dysplasies rotuliennes/fémorales. MPFL représente le stabilisateur rotulien pour 30® flexion du genou.
Matériel et méthode : Nous rapportons 25 cas MPFL opérés sous contrôle arthroscopique et RxTV (2015-2019). Le diagnostique se pose sur des signes clinique (Smilie, Fithian, Tilt signe) et radiologique (Insall Salvati, Caton Dechamps, Blackburne-Peel, les angles trochléaires). Les techniques pour la stabilisation de la rotule, améliorées au fil du temps, avec la connaissance approfondie de l’anatomie du genou, et de sa biomécanique à différentes étapes de la vie (enfant, adolescent, jeune, adulte), Ali-Krogius, Ali-Krogius-Lecene, Roux, Fevre-Depuis Maquet type I, II, III, Elmslie-Trillat, résection arthroscopique de l’aileron rotulien sexterne, plicature de celui interne, ostéotomie Fulkerson et reconstruction MPFL avec l’autogreffe. On propose la reconstruction anatomique MPFL sous guidage arthroscopique, avec l’identification du „mall-tracking” rotulien et des lésions associées en MIS, et sous fluoroscopie pour le vissage du néoligament dans le point isoélectrique femoral, avec les repères anatomo-radiologiques, après la preparation de la greffe tendineuse, hamstring” et le forage des tunnels rotuliens (parallélisme/convergence au dela, de la pression).
Résultats et discussions : On souligne l’importance de l’échographie préopératoire pour aprecier la qualitée, et la longueur de la greffe, de la RxTV pour la visée correcte du point isoélectrique fémoral, de l’arthroscopie dans l’identification des zones d’hyperpression patello-trochléaire, appréciant l’excursion rotulienne, empêchant l’hyperpression arthrogène. Conclusions : La reconstruction du MPFL est anatomique et en MIS, Corrige l’angle Q Est fiable et s`applique aux jeunes avec réhabilitation rapide.
Introduction : It occurs in active individuals (6–77/100,000/year), most often following poorly treated post-traumatic injuries, or in cases of patellar/femoral dysplasia. The MPFL represents the primary patellar stabilizer during the first 30° of knee flexion and is injured in 94% of dislocations.
Materials and Methods : We report 25 cases of MPFL reconstruction performed under arthroscopic and fluoroscopic (RxTV) control (2015–2019). The diagnosis is established clinically (Smillie, Fithian, Tilt sign) and radiologically (Insall–Salvati, Caton–Deschamps, Blackburne–Peel indices, trochlear angles). Techniques for patellar stabilization have improved over time with a deeper understanding of knee anatomy and biomechanics at different stages of life (child, adolescent, young adult, adult), including: Ali-Krogius, Ali-Krogius-Lecene, Roux, Fevre-Depuis Maquet type I, II, III, Elmslie-Trillat, arthroscopic resection of the lateral patellar retinaculum, medial retinacular plication, Fulkerson osteotomy, and MPFL reconstruction using autograft. We propose anatomical MPFL reconstruction under arthroscopic guidance, with identification of patellar maltracking and associated lesions using minimally invasive surgery (MIS), and fluoroscopic guidance for fixation of the neoligament at the femoral isometric point, using anatomoradiological landmarks. This is performed after preparation of the hamstring tendon graft and drilling of the patellar tunnels (parallelism/convergence beyond the pressure zone).
Results and Discussion : We emphasize the importance of preoperative ultrasound to assess graft quality and length, fluoroscopy (RxTV) for accurate targeting of the femoral isometric point, and arthroscopy for identifying areas of patellotrochlear hyperpressure, evaluating patellar excursion, and preventing arthrogenic hyperpressure.
Conclusions : MPFL reconstruction is anatomical and minimally invasive (MIS), It corrects the Q angle, It is reliable and suitable for young patients, allowing rapid rehabilitation.
